A Full-Service Care Management Program
The LifeCare Management Program was designed for individuals who wish to be cared for at home. It provides patients with an experienced team of healthcare professionals who will create a customized plan of care to meet the needs of the patient and their family. Patients are able to remain at home safely and comfortably, while giving family members peace of mind knowing that their loved one is receiving the best care possible.

What is a Care Manager?
A Care Manager is a professional who arranges care for individuals needing care while their family lives at a distance or is otherwise unable to provide the necessary level of care. Nursing & Home Care applies the team approach to care management, where professionals work together to coordinate the care for each patient.
How does the process begin?
Through interviews with the patient and their family, an initial assessment is made. The Care Manager will determine the needs and desires of the patient and create a customized plan of care.
Will there be someone for family members to contact?
Yes. The family will be assigned a Care Manager who will work in collaboration with a team of experts and provide updated information to the family as needed.
