| RNs: Full-time, benefited, part-time, on-call, per-visit and weekend positions. Requires current CT RN license, one year clinical experience. |
| • | Primary Care Nurses/Case Managers: Provide direct care / management of care for adult homebound individuals. |
| • | Hospice Nurses: Join a dynamic interdisciplinary team providing end-of-life care to hospice and pre-hospice patients in skilled and home settings. Long term care or facility experience a plus |
| • | HOME HEALTH AIDES: Hourly, Live-in and Shift assignments available. Paid travel time and mileage. Own transportation and minimum of six months recent experience working as a HHA or CNA or recent graduate of a Connecticut training program with certificate. |
| RN, Clinical Coordinator: Full-time, benefited position supporting Clinical Director in the supervision of clinical staff; oversight of paraprofessional department and direct responsibility for coordination of care. Provides direct care 1-2 days/week. Occasional weekend visits and demonstrated competency in the provision, delivery and administration of home health care services required. Requires current CT license and BSN, (MS/MA preferred). Minimum two years clinical experience, prior supervisory experience, competency in home care regulatory and reimbursement matters and computer literacy. |

| Billing/Collections Professional: Full-time, experienced medical billing/collections professional (home care experience preferred). Requires strong computer skills (MS Office, knowledge of McKesson a plus), communication skills and organization skills. |
| REHAB PROFESSIONALS: Per Visit opportunities available. Requires current Connecticut license and three years experience. |
